Last Minute Mother’s Day Gift for Kids

I have always loved those “All About Mom” interviews I’ve received past years for Mother’s Day, but I always wanted a picture of my child to go with it so I don’t have to visualize in my head what they looked like at the age when they filled it out. So I combined it and made it so I can stick a 4×6 picture right onto the interview so I can look at their precious faces when I read their answers and remember that cute age!
I whipped up this quick Mother’s Day Questionnaire that has room for you to paste a 4×6 print of the child you interview!
All you have to do is print out a 4×6 picture of your child:
I used my Canon PIXMA MG7720 Wireless Photo All-in-One Printer to print off a photo of Miss D from my phone. I used Canon Photo Paper Plus Glossy II.
Then print off the Free Printable: {scroll down for download link}
Then all you need is some glue or double sided adhesive to attach the photo to the top of the questionnaire! Then you need to sit your child down and ask them these 5 simple questions. Shouldn’t take more than a couple minutes!
